Advanced Health Assessment Comprehensive SOAP Note
Patient Information
Name: Jane Doe
Age: 30 years
Gender: Female
Race: Caucasian
Chief Complaint: “I have lower abdominal pain and vaginal spotting for the past two days.”
The patient is a 30-year-old female who presents with complaints of lower abdominal pain
accompanied by vaginal spotting for the last 48 hours. She reports the pain began gradually and
has progressively worsened. The pain is described as sharp, intermittent, and localized mainly in
the lower pelvic region, more pronounced on the right side. She rates the pain as 7/10 in
severity.
The patient states the spotting is light pink to dark brown and does not resemble her normal
menstrual flow. She reports associated nausea and mild dizziness but denies vomiting. She also
complains of fatigue and generalized weakness. Movement worsens the pain, while lying still
slightly relieves symptoms.
Her last menstrual period was approximately six weeks ago. She confirms being sexually active
with one partner and inconsistently using contraception. She denies fever, chills, urinary
burning, frequency, flank pain, diarrhea, or constipation.
,Because of the missed period and current symptoms, the patient expresses concern about a
possible pregnancy complication.
