1. Acid Base Balance ● PH: 7.35-7.45 (↓ acidosis ↑ alkalosis) ● Bicarb: 22-26 (↓ acidosis ↑ alkalosis) ● Ph & bicard are going the same way; it's metabolic. Different directions its respiratory. ● Acidosis & alkalosis depends on Ph. ● As the PH goes, so does my patient. Except potassium (goes opposite) ○ ↓ : body systems go down ○ ↑: every system in the body get more irritable ■ If Ph goes down, you have high potassium. ● Alkalosis ○ Irritability ○ Hyperreflexia (+3,+4: high +2: normal, +1,0: hyporeflexia) ○ Tycypnia ○ Tachycardia ○ Borborygmi: increased bowel sounds (paralytic ileus: think things are shutting down) ● Acidosis ○ Lethargy (obtunded: one step above lethargy) ○ Paralytic ileus ○ Coma ● Seizure ↑, coma ↓ (goes same as Ph) ○ Alkalosis will seize ( need suction to prevent aspiration) ○ Acidosis will respiratory arrest (need ambu bag at bedside) ● Mac Kussmal ○ Compensatory respiratory pattern
