1. Patient Demographics & Chief Complaint
• Name: Victoria Lewis
• Age: 25 years old
• Sex: Female
• Occupation: Unemployed teacher (has an education degree and is
looking for a teaching job)
• Chief Complaint: "I have a rash on my legs and arm that just
showed up and it itches a lot."
2. History of Present Illness (HPI)
Victoria presents with a new, itchy rash. The timeline and
characteristics are as follows:
• Onset: The rash began approximately 36 hours ago.
• Initial Location: It started on her bilateral inner thighs.
• Current Location: It has now also appeared on her left inner
forearm.
• Sensation: Described as itchy (pruritic), red, slightly swollen, and
somewhat tender.
• Key Exposures in the Last 2 Days (Potential Triggers):
o Went hiking and "bushwhacking"
o Sat in a hot tub
o Used a new lotion
o Ate scallops for the first time
• Relieving Factors: None reported.

other systemic symptoms.
3. Past Medical History (PMH) & Medications
• Past Medical History: Generally healthy; denies any chronic
illnesses, injuries, or prior surgeries.
• Preventative Health: Annual physicals, dental checkups, and Pap
smears (last one was normal).
• Medications: Oral contraceptive pill (one pill per day, does not
recall name or dose).
• Allergies: Ceclor (Cefaclor) – reports developing hives as a child.
4. Family History
• Mother (58): Alive and well.
• Father (59): Has hypertension.
• Siblings: Sister (21) and two brothers (35, 32) are all alive and
well.
• Maternal Grandfather: Deceased at 81 from cardiovascular
disease.
