1.1 Nursing Process (ADPIE)
The nursing process is the foundation of all clinical decision-making on the HESI. Every question that asks 'what
should the nurse do first?' is rooted in the nursing process.
ADPIE Framework
Column Description
Assessment Collect subjective & objective data. ALWAYS the first step unless
a life-threatening intervention is already identified.
Diagnosis Analyze data; identify nursing diagnoses. Prioritize using Maslow
and ABCs.
Planning Set measurable, client-centered goals. SMART goals: Specific,
Measurable, Achievable, Relevant, Time-bound.
Implementation Carry out the plan. Independent, dependent, and collaborative
nursing actions.
Evaluation Determine if goals were met. If not met, restart the process.
TEST TIP
On the HESI: Assessment ALWAYS comes before implementation unless the client is in immediate
life-threatening danger (e.g., airway obstruction). When in doubt — assess first.
1.2 Maslow's Hierarchy & Clinical Prioritization
Maslow's hierarchy guides priority-setting. Physiological needs (airway, breathing, circulation, pain) always
outrank psychological or safety needs in acute care.
Priority Order in Clinical Decisions
