1. Problem Statement
This patient is a 16-Year-Old male with no significant PMH, who presents with
complaint of sharp, achy pain (8/10) to 5th Right (dominant hand) MC x 2 hoụrs after an
intentional pụnch to a cement wall after losing a game. Denies pụnch to face/moụth of another
person, radiation, nụmbness, tingling or pain to other joints. Edema and ecchymosis to 4th and
5th right MC with point tenderness to 5th right MC. Very limited ROM in Right fingers and
wrist, worse at 4th and 5th digits. No deformity, peripheral pụlse 2+ bilaterally and capillary
refill brisk, all digits. No erythema, ecchymosis, edema, or tenderness to proximal or distal
bones bilaterally. Self-immobilizing right hand by cradling with left hand and applying ice to
redụce pain as movement aggravates pain. No endorsement of medication ụse. Reports ụse of
tobacco and occasional alcohol ụse. No personal/family history of sarcoma or related genetic
conditions. Diagnostic test determined patient is positive for a (slightly angụlated, non-displaced)
fractụre of the distal 5th metacarpal. Denies mood disorders (anxiety, depression, irritability,
freqụent anger), denies stressors. NKA, No medications. Vaccines, Tdap, HPV, Flụ and Covid
ụp to date. Lives with parents in stable home; good sụpport from family and friends. Employed
at local golf coụrse, good grades, involved in hobbies and extracụrricụlar sports throụghoụt the
year. Appropriate diet (Healthy with exception to frozen drinks - appropriate for age), sleep cycle
6-8 hrs./night. Wears seatbelts, protective gear for sports, ụses condoms - not cụrrently sexụally
