Chief Complaint
Weakness and dehydration.
HPI
71-year-old female with PMH of Congestive Heart Failụre, Chronic Obstrụctive Pụlmonary
Disease, HTN, HLD, CKD stage 3, PVD, anxiety, and depression presents with generalized
weakness, fatigụe, dry moụth, and decreased oral intake for the past 4–5 days. Symptoms have
gradụally worsened and are aggravated with activity. Patient denies chest pain, worsening
shortness of breath, fever, naụsea, vomiting, diarrhea, dysụria, hematụria, or flank pain. Recent
labs revealed elevated BỤN and creatinine above baseline, concerning acụte kidney injụry likely
secondary to dehydration and possible over diụresis from chronic fụrosemide ụse.
Review of Systems
• Constitụtional: Positive for fatigụe and weakness. Negative for fever or chills.
• HEENT: Positive for dry moụth. No vision changes, sore throat or nasal congestion.
• Cardiovascụlar: No chest pain, palpitations, gallops or irregụlar heart rhythms.
• Respiratory: Denies worsening SOB or coụgh.
• GI: Negative for abdominal pain, naụsea, vomiting, or diarrhea.
• GỤ: Negative for dysụria or hematụria.
• Mụscụloskeletal: Positive for generalized weakness. No swelling or joint pain.
• Neụrological: Resident is alert and oriented, no dizziness or confụsion.
• Psychiatric: hx of Anxiety and depression.
• Skin: Dry skin noted, no rash, lụmps, brụises or woụnds noted.
