“My grades are slipping.”
History of Present Illness (HPI)
Avery Brook is a 10-year-old female brought to the clinic by her mother due to
worsening school performance over the last 4–6 months. Mother states Avery was
previously doing well academically but recently began struggling with
assignments, forgetting homework, and receiving poor grades.
Teachers report:
Difficulty maintaining attention in class
Frequent daydreaming
Easily distracted behavior
Incomplete classwork
Poor organization
Forgetfulness
Mother reports Avery:
Snores loudly at night
Appears restless during sleep
Has trouble waking up in the morning
Seems tired during the daytime
Becomes irritable after school
Has less motivation toward school activities
, Mother also notes recent emotional stress related to parental separation
approximately 6 months ago. Avery has made negative self-comments including:
“I’m dumb.”
