**2. Chief Complaint (CC)**
* **"Pain when I pee"** / Dysuria
* **Reason for Encounter:** A 28-year-old female with a 2-3 day
history of progressive dysuria, urinary frequency, urgency, and
suprapubic discomfort .
**3. History of Presenting Illness (HPI) - OLD CARTS**
, * **Onset:** Symptoms started approximately 48 hours ago (2 days)
and have been gradually getting worse .
* **Location:** Sharp, burning pain located in the urethra ("right at
the opening where the pee comes out") and a dull ache in the
suprapubic area (lower belly) .
* **Duration:** The burning sensation lasts for the entire duration of
urination and lingers for a few minutes afterward. The suprapubic ache
is more constant .
* **Characteristics:** The pain is described as a sharp, stinging, or
burning sensation. The urine is reported to be cloudy and have a
stronger odor than usual .
* **Aggravating Factors:** Urination is the primary aggravating factor.
Holding urine for too long also worsens the bladder pressure .
* **Relieving Factors:** Drinking plenty of water provides mild relief
(likely by diluting the urine). A heating pad applied to the abdomen
helps with the pelvic pressure but does not relieve the burning .
* **Timing:** Symptoms are consistent throughout the day every
time the patient attempts to void .
* **Severity:** The pain during urination is rated as a **7/10**. The
constant suprapubic pressure is rated as a **3/10** .
* **Associated Symptoms:** Patient reports urinary frequency (every
30 minutes) and urgency. She denies gross hematuria, vaginal
discharge, itching, flank pain, fever, chills, nausea, or vomiting .
