HISTORY OF PRESENTING ILLNESS (HPI) –

Onset: Symptoms began approximately 2 to 3 days
ago. The patient reports that the discomfort started
mildly but has progressively worsened each day.
Location: The pain is localized to the urethral area
and suprapubic region (lower abdomen, just above
the pubic bone). She denies any flank, back, or
vaginal pain.
Duration: The burning sensation occurs primarily
during urination and lingers for 2-3 minutes after
voiding. She feels a constant urge to urinate,
needing to void every 30 to 60 minutes during the
day and 2-3 times at night (nocturia). Each void
produces only small amounts of urine.
Character: The patient describes the pain as a
"sharp, burning, glass-like" sensation that is most
intense at the end of the urinary stream. She also

reports a feeling of suprapubic heaviness or
pressure.
Aggravating Factors:
• Urination (especially at the end of the stream)
• Drinking coffee, soda, or caffeinated beverages
• Prolonged periods without voiding
• Sexual intercourse (mild discomfort, though she

Relieving Factors:
• Drinking more water (dilutes urine and slightly
reduces burning)
• Applying a heating pad to her lower abdomen
• Taking over-the-counter AZO (phenazopyridine)
