Type 2 Diabetes Mellitus (T2DM) represents a long-term metabolic illness which develops through
insulin resistance and deteriorating beta-cell function alongside elevated blood sugar levels. The
condition causes cardiovascular disease and three major complications: nephropathy and retinopathy and
neuropathy.
I chose T2DM because it affects many people across different life stages thus requiring skilled drug
management for various groups such as children and pregnant women and senior citizens.
b) Describe the prescribing recommendations from the CPG and how these could impact
pediatric, pregnant, and older adult populations considering pharmacokinetic principles
The 2025 ADA Clinical Practice Guidelines contain specific medication recommendations for the
management of diabetes.
First-line therapy:
The primary medication choice for most adult patients should be metformin.
SGLT-2 inhibitors and GLP-1 receptor agonists should be selected for patients who have cardiovascular
disease or heart failure or chronic kidney disease (CKD).
The medication selection process follows the ABCD framework which includes Age factors and Body
weight factors and Complications factors and Duration of diabetes factors.
Glycemic targets:
The recommended A1C target for non-pregnant adults should be below 7%.
Healthcare providers should set A1C targets below 8% when treating older adults or patients with
additional health conditions or those with restricted life expectancy.
