On the initial home visit, the nurse should inspect the environment. The nurse should note the
cleanliness of the house, the safety of the living conditions, and the family dynamics in the home.
If the home is messy, to a certain level, it can hinder the patient’s health outcomes. Assessment of the
living conditions is pertinent because messiness can lead to unsanitary surfaces and more susceptibility
to dust or allergens that can exacerbate the patient’s condition. Assessing safety would be important to
eliminate falls from cords or fires from frayed wires. The home health care nurse should do a full head-
to-toe assessment on this client that includes neurological assessing since the patient has a history of
seizures. The nurse should also engage in conservation with the patient to see if the patient fully
understands his diagnosis, medication regimen, and if he has any questions.
Teaching that should be extended to the patient’s wife and daughter would be when to
administer the medications, if any medications can be taken with or without food, and what side effects
or adverse reactions to looking for in the patient. I would instruct the family to administer the medication
routinely each day to eliminate another seizure from occurring. Side effects that the family should be
advised to look for are confusion, dizziness, constipation, and a rash. Also, instruct the patient and his
family to be mindful of no alcohol consumption or cigarette smoking . Also, let all parties
involved know that it’s extremely important to follow up routinely with the physician especially for blood
work monitoring.
The nurse should immediately help the patient down the floor as if the patient was experiencing a
fall. The nurse should immediately make sure the patient’s head is elevated with jacket or blanket or just
