Social Determinants of Health The goal of the Social Determinants of Health is to create environments in our social and physical surroundings that promote health for everyone. Healthy People 2020 has identified five domains for healthcare workers to assess and address for optimum health promotion: economic stability, education, social and community context, health and healthcare, and neighborhood and built environment (Office of Disease Prevention and Health Promotion, 2020-d). The most effective tool for evaluating a patient’s needs within each of the five domains is the Comprehensive Health Assessment. Proper assessment of these needs will provide accurate information for an individualized plan of care with goals that are attainable for each patient.
