Social Determinants of Health The comprehensive health assessment plays a vital role in establishing care and forming a relationship between the patient and their provider. Not only can the assessment reveal areas of the patient’s physical well-being that need improvement, but also any barriers they may face in accessing care. The comprehensive assessment provides a holistic picture of the patient so the provider can make an informed decision in their treatment plan. For the comprehensive health assessment of Nikki, interview techniques will be discussed and how they were used to assess her social determinants. The influence of the patient’s economic stability and educational status on her access to healthcare will be discussed as well. Her subsequent access to healthcare will be analyzed for its impact on her quality of life and how her environment/neighborhood influences her access to care. Finally, the importance of assessing the patient’s social determinants will be discussed and used to form a nursing care plan.
