OBJECTIVE (S): CC: “Patient is here for his annual well-child 4-year-old visit, the mother states patient speech is not where it should be and would like a speech evaluation, the patient has had a speech delay since age 1, started out in ECI therapy with the speech that ended at age 3, mom states she has made several attempts to find a speech therapy that takes her insurance, but everyone takes Medicaid.” HPI: 4 years-old patient is here for his well-child exam. The patient’s mom is concerned about his speech progress. The mother states he currently sounds like a 2-year old does not make complete sentences. Concerned about not being able to articulate himself and feels like he is not ready to move into the PK4 classroom at school
