NURS 310 Psychiatric Soap Note 001: ELISSA HENDRICK Subjective Patient presents complaining of weight loss, fatigue and loss of interest in activities. 16 year-old female with reported hx of social anxiety and phobia, mainly related to her weight, has never been formally diagnosed/treated for these conditions. Patient reports that she feels overweight and has significantly reduced her caloric intake because of this. Patient reports utilizing laxatives, and inducing vomiting voluntarily. Patient reports excessive exercise. Patient reports recently experiencing amenorrhea and fracturing her left foot, required to wear a walking boot for 8 weeks. Patient reports the only medications she takes are over the counter laxatives. No known allergies. Reports that parents are divorced and that she lives with mom, noting that mom is the reason she came in to be seen. Include chief complaint, subjective information from the patient, names and relations of others present in the interview, and basic demographic information of the patient.
