The client was referred by her father for the management of the following presenting complaints:
• Confused dreams with reality
• Stole something even when she had the cash
• Been blue

• Saw the train moving while sitting still
• Hard to stay in one place
• Felt like no bones in the hand
• Zoned out very often
• Chased a bottle of aspirin with a bottle of vodka possibly to kill herself
• Headaches
• Felt sad
• Was seeing things
• Attempted Suicide but says that she did not
• Promiscuous
History of Presenting Problem:
This information could not be obtained.
Personal History:
The client did not mention anything related to problems at the time of birth or any other
prenatal history. No mention of unusual events, head injuries, trauma or problems during
development.
In one session her mother randomly mentioned of a time when S.K. was a baby and her
mother was changing her diaper. She looked back to grab some powder and S.K. rolled off and fell
down the bed. She broke her leg so the doctor put her in a body cask and also strapped her down.
They had a trip planned around that time so they took her with them strapped on the back seat.
Client had no memory of this.
