Personal Goal: I have battled with depression and anxiety for years in which I have felt stuck in

my life not knowing my purpose. I lost my job back in November and was in a dark place. As of

lately, my personal goal is to get back to a healthy mental state of mind. Recognizing my

negative feelings and thoughts and turning them into positive ones, healing my inner child, and

evolve into the woman I want to be. I have incorporated positive habits and routines to help me
overcome any more hardships life will throw me. Some of those habits/routines are meditating,
exercising, journaling, reading, affirmations, visualizations, and practicing gratitude. I have

become aware of my not so good feelings and how to turn them around. My current book I am

reading is You Are A Bad Ass, Daily Teachings by Rhona Byrne and next I will read The Power
of Positive Thinking by Norman Vincent Peale. This is only the beginning of my new life and I
know there will be days I won’t want to do any of the above but I have to remember “My Why”
and push through those feelings. I am feeling better than I have in years with everything that I

Academic Goal: I have been on this journey of trying to obtain my bachelor’s degree for 20
years. I have 10 classes before I will have my degree which I am very excited about. This time

, Professional Goal: I just accepted an entry-level position as a Patient Service Representative
from being unemployed for 8 months. I am positioning myself for a Human Resource role in
Benefits or as a Human Resource Generalist which are my short-term goals. I am doing this by
applying for those specific roles and signing up with staffing agencies to assist me. I want to
increase my professional network with individuals who are in HR leadership roles to assist me
with reaching my goal. I plan on joining SHRM and attending local events for HR Professionals
in my area. I am also part of several HR groups on Facebook and have connected with a lot of
professionals on LinkedIn. For my long term professional goals, I am going to one day become
Director over a Human Resource Compensation and Benefits department as well as open a

The goal was to approve the claim within 7 days; however, it took more than 7 days to receive

medical documentation from the doctor’s office to verify if the employee was disable and unable

to perform their job. The doctor’s office was not getting the medical records request that I was
faxing or taking too long to respond to the request. I suggested to management to do a trial run
to obtain verbal confirmation of the employee’s medical condition and treatment plan over the

phone with the doctor’s office to cut down on the days it would take to receive the actual

medical records. With consent from the employee, I was able to contact their provider, obtain

the needed information over the phone, document the system, and approve the claim within 2
