Posttherpetic Neuralgia & Neuropathic Pain: Assess location, characteristics, and intensity, of pain periodically during therapy. 4. Migraine Prophylaxis: Monitor frequency and intensity of pain on pain scale. 5. Restless Leg Syndrome: Assess frequency and intensity of restless leg syndrome prior to and periodically during therapy. Instruct patient to take medication exactly as directed. Patients on tid dosing should not exceed 12 hr between doses. Advise patient not to take gabapentin within 2 hr of antacid. 3. Advise patient and family to notify healthcare professional if thoughts about suicide or dying, attempts to commit suicide, new or worse depression, anxiety. 4. Advise female patients to notify healthcare professional if pregnancy is planned or suspected or if breast feeding. 5. Advise patient to carry identification describing disease process and medication regimen at all times. Caution patient to avoid taking alcohol taking alcohol or other CNS depressants concurrently with this medication. 4. Emphasize the importance of follow- up examinations to determine effectiveness of the medication. 5.Seizures: Patients on anti-convulsant therapy should carry identification describing disease process and medication regimen at all times. Maintenance treatment of bipolar I disorder (with lithium or divalproex) 5. Adjunctive treatment of depression. Assessments/ Monitoring: 1. Monitor mental status (mood, orientation, behavior) before and periodically during therapy. 2. Assess for suicidal tendencies, especially during early therapy. 3. Monitor for development of neuroleptic malignant syndrome (fever, respiratory distress, tachycardia, seizures, diaphoresis, hypertension or hypotension pallor, tiredness) Notify health care professional immediately if these symptoms occur. 4. Assess for rash periodically during therapy. May cause Stevens- Johnson syndrome. Discontinue therapy if severe or if accompanied with fever, general malaise, fatigue, muscle or joint aches, blisters, oral lesions, conjunctivitis, hepatitis, and or eosinophilia. 5. Monitor for signs of pancreatitis (nausea, vomiting, anorexia, persistent severe abdominal pain, sometimes radiating to the back) during thereapy. Side Effects: CNS: 1. Advise patient and family to notify health care professional if thoughts about suicide or dying, attempts to commit suicide, new or worsen depression, new or worse anxiety, feeling very agitated or restless, panic attacks, trouble sleeping, new or worse irritability, acting aggressive, being angry or violent, acting on dangerous impulses, an extreme increase in activity and talking, other unusual changes in behavior or mood occur. 4. Refer patient for nutritional, weight, or medical management of dyslipidemia as indicated. 5. Instruct patient to notify health care professional promptly of sore throat, fever, unusual bleeding, or bruising, or rash.
