Strep Throat-Antimicrobials case study

CASE SUMMARY: Five-year-old male patient, David Jacobs, appears at the pediatrician office with chief complaints of a sore throat. Patient’s mom states that the patient has been sleeping more than usual, running fever, and refusing to eat any solids for the past two days. They both deny any coughing, shortness of breath, or vomiting. Patient does report having a headache and stomach ache. Mom denies any family illness recently but does state that many parents at the patient’s daycare have been reporting strep. On exam patient is noted with fever of 38.8C (101.8F), erythematous tonsils with white exudate, edematous uvula, petechiae to patient’s soft palate, and enlarged anterior cervical lymph nodes. Symptoms (List two pertinent signs and symptoms):  Sore throat  Fever of 38.8C (101.8F)  Sleeping more than usual  Refusing to eat solid foods x2 days  Complains of head and stomach ache  Enlarged anterior cervical lymph nodes  Erythema of tonsils with white exudates  Edematous uvula  Petechiae to soft palate  Patient attends daycare where parents are reporting strep Recommended Drugs: List three recommended drugs as indicated for this diagnosis (one drug from 3 different drug classes).  Amoxicillin 40 mg/kg/day to be given by mouth 2 times per day in equally divided doses for 10 days for pharyngitis streptococcus (strep throat).  Zithromax 12mg/kg/day by mouth once daily for 5 days for pharyngitis streptococcus (strep throat).
