Louella Mesina’s Synopsis of the Shadow Health Assignment
This paper is to discuss the Assignment 5.2: Shadow Health: Focused Exam: UTI with Antibiotic
Sensitivity. This case study is regarding Makayla Henderson, a 21-year-old biracial woman, who
has come to the university health clinic complaining of urination issues. This assignment was a
focused exam on Makayla who has current sign and symptoms of a Urinary Tract Infection
(UTI) and antibiotic sensitivity. The goal of the assignment was to conduct a thorough interview
with Makayla utilizing therapeutic communication to help determine the appropriate
pharmacological therapy for her UTI. Then, using the subjective and objective data, I discovered
during the interview, that the next step was to determine an appropriate course of treatment
(pharmacological and nonpharmacological techniques). After examination, pharmacological
treatment of an antibiotic called Nitrofurantoin was determined to treat her UTI and,
Phenazopyridine, for pain associated symptoms. I then proceeded to educate Makayla on why
Nitrofurantoin was most appropriate antibiotic medication due to lab results and educate her on

Reflection of Experience Learned

This assignment helped me hone my interview skills with Makayla. I focused on utilizing
therapeutic communication to gather subjective and objective patient data. I was able to gain
comprehensive data on Mikayla’s current and past medical condition/medical history.

Performing a good health history is challenging. I found myself trying to find the best way to ask

in a therapeutic way. Sometimes, it was frustrating trying to keep on track because filling in the

gaps of what Makayla shares versus what I needed to make a good assessment. I tried my best to
clarify my questions while practicing on documenting the data. I made sure I was documenting
my notes and that I was hearing Makayla accurately. I practiced using professional terminology

but found it difficult to also remember to document my objective observations simultaneously.

The take away I learned from this experience is that it takes a lot of therapeutic communication
to build rapport with patients and that I need to be a better listener and focus on the details better.

This assignment helped me realize that there are so many important considerations and principles
when prescribing antimicrobial therapy. The most import aspect of prescribing any antibiotic is

obtaining an accurate diagnosis of infection. We should understand the difference between a viral
versus a bacterial cause of infection and to be able to identify situations when antimicrobial
therapy is not needed. We should know how to weigh the benefits against the harms of
antibiotics according to the patient's needs, signs and symptoms. Then, it is important to note,
while investigating and identifying opportunities to switch to narrow-spectrum, we should try to
find the most appropriate and cost-effective oral agents for the shortest duration to encourage
