For my policy priority I decided on patient safety and how poor staffing and nurse
burnout affect it. Patient safety should be the number one priority and concern of all
medical facilities and professionals. Every facility will have policies in regards to patient
safety however, their nurse to staff ratio and work environments to prevent burnout are
insufficient. There are also those policies related to patient safety that are not in
conjuncture with reality, such as who qualifies for a 1:1 sitter. At my current hospital, a
very large hospital system that spans over Central Florida, they just had budget cuts and
laid off a rather large number of patient care techs and will no longer allow nurses to get

overtime, overtime which was always available only because nurses were needed for the
amount of patients we have. Now, if a patient needs a 1:1 sitter the techs are no longer

available to watch them however nurses are still held responsible if the patient falls. Also,
nurses are now being tripled on a daily basis. To make it worse, they are not tripling these

nurses with the patient’s rooms right next to each other; the patients are spread out and

even on the other side of the floor. According to Cho et al. (2015), an inadequate working

environment affects both nurses and patients in a negative manner; nursing burnout and
turnover increases, patient satisfaction decreases and patient mortality rates increase.
I believe incrementalism is the model that would be best applied to my policy. I

say this because there are safety policies already in place however; these policies need to

be updated in order to correspond with what happens in real life. It is better to make small

changes in already existing policies rather than completely overhaul them all at once.
