Urinary System Chapter 45
Unit function of kidneys= nephron
300 million of nephrons in the kidney
Bowman capsule – where filtration takes place**
CVA – costal vertebrae angle – where kidney’s are located
Testing kidneys: BUN, Creatinine(most definite), GFR, I&O(normal output = 30-
40ml/hr)
Blow test – when pt complains of tenderness
What does kidney do? Excretes nitrogen(protein, ammonia – in form of urea) from
the body. Cleans/produces erypoietin hormone - blood. Helps regulate BP(by renin
angiotensin system) /calcium level in body by activation of calcitonin hormone –
form of vitamin D – that causes absorption of Calcium from the stomach. Regulates
electrolytes.
Kidney Disease – anemic patient
Dialysis – gets synthetic erypoitein 3x a day for the rest of their life
Landmarks
Adrenal Gland – on top of the kidney
Nephron is a function unit –= microstructure; glomerulus, bowmans capsule,
tubular system- secretion of urine
Ureters – join the pelvis at the UPJ – ureteropelvic junction, joins the bladder at the
ureterovesical junction (UVJ) about 250ml, you will feel the urge to pee. If urine is in
bladder too long – bacteria will grow developing infection**
Kidneys – have their own blood supply, passes through the bowman’s capsule ,
normal GFR level= 125ml/min = normal
Urethra – shorter in female(UTI), longer in men, extends for the bladder
Bladder: serves as a reservoir, urination micturition, voiding. It is a muscle. Capacity
600 – 1000ml
Prostate Gland – men; most adults it will get enlarged – impeding urinarty flow.
Monitor older adult for urinary function
Six and wt of kidney decrease with age
Loss of 30%-50% of glomelur function
Decreased GFR rate and blood flow in older adults
Affected hormone level and loss of elasticity of bladder
Urine retention – bladder will be distended and swollen – straight cath * neurogenic
bladder**
Objective Data – kidney punch (blow test), percussion, palpitation, ausculate (CVA)
