1 Week 9 Decision Tree Samantha Zeisloft Walden University NURS 6640 Dr. Letterio 2 Week 9 Decision Tree This week’s case study follows Katie. She is an 8 -year-old Caucasian Female and is brought to the office by her parents (Laureate, 2021). They were referred by their primary care provider after Katie’s teacher suggested she be evaluated for ADHD to him (Laureate, 2021). The PCP referred them to psychiatry to evaluate Katie (Laureate, 2021). The teacher filled out a “Conner’s Teacher Rating Scale -Revised” form and it states that Katie is “inattentive, eas ily distracted”, she easily forgets things, has issues with spelling, reading and math (Laureate, 2021). She has a short attention span and tends to pay more attention to the things she is interested in (Laureate, 2021). Her teacher believes that she is bored in school and is not interested in the work (Laureate, 2021). She is easily distracted and rarely finishes things she starts and will occasionally not follow through on instructions and fails her homework (Laureate, 2021). Katie’s parents do not believe that she has ADHD and believes that she would be running around like a crazy person and she would be defiant or have temper tantrums (Laureate, 2021). Katie doesn’t believe there is a problem and states that she doesn’t know what the “big deal” is and thinks school is okay (Laureate, 2021). She enjoys learning about art and going to recess but she doesn’t like the other classes and sometimes they are hard, and she gets lost in the lesson (Laureate, 2021). Her mind does wander during class a nd thinks of more interesting subjects (Laureate, 2021). Her teacher will call her name and she won’t know what was asked or the subject being discussed (Laureate, 2021). Her mental status exam shows that she is dressed appropriately for the appointment. S he has clear, coherent, and logical speech (Laureate, 2021). She is alert and oriented and reports that her mood is euthymic (Laureate, 2021). She has no mannerisms, gestures or tics that stand out during the interview (Laureate, 2021). Her attention and concentration are intact as evidenced by
