Henry Williams, Part 2 henry 2

Guided Reflection Questions
Opening Questions
How did the simulated experience of Henry Williams’s case make you feel?

This was a more difficult scenario because it was more related to mental health and assessments that I
have not had a ton of experience doing. It was hard to also remember that in order to transfer a patient,
you should do a full set of assessments over again, so the facility has the most accurate information and
up to date information about the patient. Not only did I have to focus on Henry, I also had to focus on his
wife and her condition.

What nursing actions did you feel were appropriate within this scenario?

Focusing on the entire patient, including his family life I think was most appropriate. This allowed me to
more understand why Henry scored what he did on his assessments, because they are directly related to
how he feels about his wife’s condition.

Scenario Analysis Questions*
PCC What communication techniques could be used to decrease the anxiety of Henry Williams and
his daughter-in-law, Betty, about his wife, Ertha?

I think the best approach was directing my communication to the family as a whole and not just
to Henry, this made sure the family and Henry knew I was concerned about all aspects of his
care.

T&C/I How would you use the situation-background-assessment-recommendation (SBAR) format to
communicate transition of care from the acute care nurse to the pulmonary rehabilitation
nurse?

During his stay, he showed signs of
depression, withdrawal, anxiety, and sleep
deprivation.

Background Patient has had a decrease in improvement
with respiratory rehab. He lives at home with
his wife who shows signs of decreased short
term and long-term memory. Patients
daughter is assisting in care for Henry and his
