The Need for Discharge Nurses for CHF Patients

Abstract
CHF patients are not receiving adequate diagnosis and treatment education during their discharge
process. Currently, this teaching process is completed at the actual time of discharge. These
patients require frequent follow up visits, as well as extensive education on lifestyle changes that
need to be made. These changes can be overwhelming to a patient, those who are not supported
and educated properly have a higher chance of not following the much needed treatment
regimen. This lack of education and non-compliance by the patient leads to higher readmission
rates. The plan to address this issue is to provide education to CHF patients who are already
diagnosed and those who are newly diagnosed. This will be completed by a discharge nurse,
beginning at time of admission and completed daily. The first step in this process was to meet
with the managers to obtain their consent. Once this consent was received, the second step was
to preform evidenced based research that consisted of reviewing 30 peer reviewed journal
articles that concentrated on the improvement of readmission rates and outcomes in CHF
patients, which was reviewed with the stakeholders. The third step, meeting with Human
Resources to evaluate the budget for hiring the needed staff. The fourth step involved the
stakeholders being consulted to provide their input and to allow them to review the evidence.
The stakeholders reviewed the budget and approved for the development of the proposal. The
final step, handing the completed proposal to the organization to be implemented.
