Many people develop worldviews based on different things that they have seen or done and chose to incorporate that into their lifestyle as something they wish to adhere to or uphold as a belief. These views aid a person in lifestyle changes such as work ethic, family life, and religion. I will explain my world view, select a nursing theory that best fits the world view that will be used to approach patient care, provide an example of how the nursing theory and worldview have been applied to practice, and explain how the nursing theory and world view will assist in future practice. Personal Worldview According to Truth Encounter Ministries (2015), a personal worldview is a set of beliefs that are from the heart that drives behavior such as how a person thinks, feels, and acts. My personal worldview is to be kind to others, and to show compassion and understanding. Kindness is an act that can speak volumes about a persons character and helps show how they care for others. DeNisco and Barker (2016) defines kindness by stating that it “involves the quality of being considerate and sympathetic to another’s needs” (p.692). When a person performs an act of kindness and does not expect anything in return, they have the potential to be a happier, kinder person (DeNisco & Barker, 2016). I recognize my religion as Christian. I was raised in a house where Christianity is the predominant religion. My father is a non-practicing Catholic but did not encourage his children to believe in Catholicism. My mother is Christian and strongly encouraged and raised us as such. Regardless of my religion, kindness is an act that is encouraged with all religion. I feel that my belief in God can help when caring for patients as I am able to take a holistic approach and understand that each patient is different and has different needs from others, even if they have the same medical problems. In order to take a holistic approach to patients, I must be able to demonstrate kindness, compassion, and an understanding of the individual and their specific needs. Nursing Theory I feel that Betty Neuman’s Systems Theory will best fit my personal philosophy of practice and approach to patient care. When caring for a patient, a person must demonstrate a certain level of compassion and that means seeing the patient as one person, not a set of diagnoses. It means looking at the patient versus reading results and basing care on that. Neuman’s theory is designed to provide a comprehensive and holistic approach to patient care that focuses on the patient and potential stressors that can influence a patient outcome (McEwin & Wills, 2014). Her approach also includes the use of primary, secondary, and tertiary nursing prevention intervention to maintain wellness for the patient (N, 2016). N (2016) explains that a person should be viewed as a unique individual with their own set of characteristics that can have stressors that another individual may not experience. Neuman’s theory focuses on primary, secondary, and tertiary prevention. Primary prevention is prevention that is focused on keeping stressors from having a detrimental effect on the body. Primary prevention also includes promoting and maintaining health wellness. (N, 2016). Secondary prevention occurs after the stressor has affected the body and focuses on preventing further damage by removing the stressor. Tertiary prevention offers the patient support once a stressor has affected the individual. Neuman’s theory also identifies that the patient is a multi-layered individual that consists of physiological, psychological, sociocultural, spiritual, and developmental (N, 2016). Her theory encompasses all different entities that can alter how a patient’s health can be affected. Current Practice As a home health nurse, I have experienced situations where patients have the same diagnosis, but the outcome may differ due to the different stressors they may encounter. One incidence involves two patients, both had left total hip replacements. Both patients had approximately the same size incision with staples that I removed. One patient’s incision healed without complications, while the other patient had fluid build up behind the incision and required a drain to be placed for four weeks. The first patient with the incision that healed lived in an upper-class area and had the support of her husband who was able to cook food and clean while she was recovering. The other patient also lived in an upper-class area but did not have any support to help her with things such as bathing, cooking, and cleaning. I was able to have a home health aide assigned to her care to help with showering assistance, but she was unable to avoid bending and twisting as she had to perform her day to day tasks. Due to the increase in movement, this caused the incision to open, causing an infection to the hardware. The patient had to have the hardware cleaned, and a drain was placed. As the nurse on both cases, I had to take into consideration the lifestyle stressors that possibly affected the second patient. Applying Neuman’s theory and understanding that although the situations were similar, there were variables that were different and altered the outcome of the patient.
