Assignment 1: "Captain of the Ship"
Introduction
The objective of this assignment allows the future PMHNP to demonstrate the ability to
collect pertinent clinical data, synthesize the information, and formulate an individualized
treatment plan for a client with symptoms of depression. Symptom variation including duration,
timing, and presumed etiology provide the information necessary for specifying a depressive
disorder in the DMS-5 (American Psychiatric Association [APA], 2013). Mindfulness is
imperative during client data collection, as abbreviated or minimal history taking may lead to
inaccurate diagnosis (Sullivan, 2019).
Treatment Plan for
B.G. Chief Complaint: “I came here because of my
depression.”
HPI: B.G. is a 56-year-old male with a reported history of unspecified depression, Autism
Spectrum disorder, and Borderline PD. He reports feeling chronically “depressed and lonely.”
He continues to endorse suicidal ideations with thoughts to “jump in front of a moving car.”
Onset of suicidality was reported as 2 days ago. He has not act upon these thoughts or engaged
in any self-harming behaviors. No contributing stressors are attributed to worsening psychiatric
symptoms. B.G. denied any thoughts to harm others or feelings that he is unable to cope with
being on an open unit for treatment. He denied hearing any voices or having any visual
disturbances. Sleep is reported as “I can’t fall asleep and I never have energy.” B.G. endorses
low motivation and lack of energy. Appetite has been unchanged, no significant weight
changes.
B.G. sought safety at LGH ED and was amenable to voluntary admission for psychiatric
stabilization and continued safety of self.
Current Psychiatric Treatment: Outpatient psychiatry for medication management at LBH.
