Student Charge Nurse/Clinical Leader Documentation
Directions:
This Direct Patient Care Documentation must be completed for one patient whom you are providing direct care in a clinical learning setting. All information within
this packet must be handwritten, (with the exception of the reflection journal) reviewed with your faculty on your assigned clinical day and submitted within 24 hours
(or as directed by course coordinator). If additional space is needed, please use the back of each page.
Student Charge Nurse/Clinical Leader Role:
defined as any student who is leading the daily activities of the unit supporting their peers and assisting with staffing needs. As the student charge nurse/clinical leader,
collaborate with your faculty/pre-licensure preceptor to determine which patients (maximum five) your peers are assigned to. Throughout the clinical experience round on
your peers to review medications/preparation (if applicable) and determine if any assistance is needed with various tasks.
• Only one student is assigned per clinical learning to this role and will discuss their experience during post-conference/debriefing.
• If this role is being performed with a pre-licensure preceptor, the student charge nurse/clinical leader will focus on the patients assigned to

their pre-licensure preceptor.
• Grading: Evaluated as Satisfactory, Unsatisfactory or Needs Improvement on the Clinical Learning Evaluation. Satisfactory rating meets the following:

linical Learning Competency: Completes all clinical learning experiences and requirements successfully. (PO 5)

•P erformance Descriptor: Completes all assignments related to the clinical learning experience within established guidelines.

• Patient Report and Information:
As the student charge nurse/clinical leader, receive report from your peers and perform an assessment on each patient. After performing an assessment on each patient,
collaboratively discuss with your peer (assigned the patient) and/or faculty/pre-licensure preceptor, which patient is the priority, their acuity, what orders or activities
could be delegated and to whom and if the patient could be transferred out of the unit or requires discharge planning that could be performed.
• Leadership Concept Map:
Develop one concept map reflecting your leadership style you demonstrated today or aspire to have. You will use the concept map and peer evaluation responses to
develop your reflection journal for this clinical learning. Areas of the concept map may be completed prior to the clinical learning experience. Common examples of
leadership styles to review: autocratic, cross-cultural, democratic, facilitative, laissez-faire, strategic, transactional and transformational.
• Peer Evaluation:
During pre-conference/pre-briefing distribute the peer evaluation to each of your peers and ask them to reflect on your leadership style throughout the clinical
learning. The peer evaluation is due by the end of post-conference/debriefing. These evaluations should be anonymous.
– Make enough copies of the evaluation for all your peers.
– Distribute the evaluations and provide a folder in a pre-determined location or with your faculty member.
– Instruct your peers to leave their completed evaluations in the provided folder by the end of post-conference/debriefing.
– Once you have all evaluations, review each after leaving the clinical learning.
– Calculate your average score for each question.
– You will use the peer evaluation results and the leadership concept map to develop your reflection journal.
• Reflection Journal Questions:
Complete a reflection journal and submit to your faculty within 24 hours of completing your clinical learning experience. Reflective journaling provides a format
to share your knowledge, skills, experiences and personal reflection related to concepts and strategies learned throughout your program. The reflection journal is
required to be typed, Word document, Times New Roman 12-point font. Minimum of one page and no more than three pages.
– After reviewing your peer evaluation results and calculating the average score for each question, identify the top two areas for improvement. For each of these
areas, discuss three ways you could improve your leadership style. Refer to specific examples and areas of opportunity encountered during your clinical day.
– Describe how your personal goals will assist with achieving your desired leadership style.
– Think about your experience as a student charge nurse/clinical leader. If you had the opportunity to do one thing over or change your leadership style,
what would it be and why?
