What are the priority nursing actions you would anticipate implementing when
caring for Mr. Jones and why?
It appears that Mr. Jones has increase fluid volume that is affecting he
respiratory status as well as his cardiac system. A Mr. Jones nurses, my first priority
would reposition him into a high fowler position to maximize proper ventilation.
Next, I would continue to monitor his BUN and Creatinine levels as increased fluid
volume has the potential to cause kidney complications. I would them continue to
assess Mr. Jones breathing with respiratory rate as well as SpO2 levels on 3L O2
NC. If he continues to demonstrate low SpO2 reading I would then instruct patient
on proper breathing techniques. Lastly, but certainly not least, I would assess pain,
as he states he has chest pain.
A low priority action would be the assessment of his foot wound. Ensuring
that the dressing is attended to, and proper infection precautions are taken place.
I would also continue to monitor further signs of infection such as elevated WBC,
fever, increased drainages, increased erythema as well increased pain.
Reflection:
In the video the nurse mentioned calling the physician about Mr. Jones
unimproved respiratory status. With a solution of provider prescribing furosemide, a
Lasix to improve the increased fluid volume. In my respond I did not think about
reaching out to the provider, however that is a very important action that needs to
be noted. Utilizing other disciplines once you have exhausted your available
options or not having any improvements is really important.
I was accurate when stating, monitor kidney functions by the BUN and
creatinine. It is even more important to monitor as Mr. Jones has a PMH of CKF.
