things can change rapidly. Evidence-based practice and new research models are proven to
provide optimal care to patients and sustain life. With new research comes a reciprocal need for
changes within the clinical setting to improve patient satisfaction and the quality of nursing care.
The need for change has been recognized on an Intermediate Cardiac Care Unit by management
after multiple complaints that a new type of report is needed. The type that is currently taking
place is very unique and is referred to as a ‘silent report’. Through feedback it was found that
nurses feel they do not know their patients adequately and are unable to properly prepare for
their shift or ask questions in order to provide quality, efficient and safe care. This paper will
serve to address the need for change, provide a salutation for change as well as utilize supporting
evidence of Lewin’s Change Theory in order to facilitate change within a healthcare
organization.
Identification of Change
The proposed recommendation to serve as a solution to this problem would be bedside
shift report. The goal of this change would be to help ensure the safe handoff of care between
nurses by involving the patient and family. This type of report can improve patient safety, quality
of care, improve the patient’s experience of care provided, improve nursing staff satisfaction and
increase accountability between nurses. The process of bedside shift report consists of the
outgoing and incoming nurse meeting outside of the patient's room, not at the nurse’s station, and
communicating the baseline information of the patient. This also provides the opportunity for the
outgoing nurse to close the relationship that was formed with the patient and introduce the
incoming nurse.
