I am here to give you some buzz words so that when you are reading this test and you are reading the questions that you really, really, understand what you're looking at and trying to pick out the right things and the right words that you need in order to answer the questions. 00:34 What this video is is pretty much compiling a lot of that basic information that you'll need for the exam. So it's going to be a lot. But that's okay. 00:46 Hopefully... And I tell everyone this... Hopefully, before you have... or while you are listening to this... you are also studying. So you are reading some material, whatever that material is, of course I do have my favorites. But you are also reading along and studying everything. Because, clearly, I just want to make this like a crash course, okay? So, this is going to be a crash course of that basic information. But I want you to continue reading and studying so you feel as confident as you can because it is an exam. And how confident are we, really, when we go into it? So that you feel comfortable knowing the material. And that's going to be the most important. 01:30 I'm going to go over things and I may give you some new ways to look at questions which is always the point, again, looking for those buzzwords so you know how to answer the questions. 01:44 So let's get started. 01:46 There will be moments where I stay in sections and it may feel like I go back to a section really quick, completely done on purpose, and sometimes not. 01:56 We're going to go ahead and get started. 02:00 Let's talk a little bit about some skin. 02:03 Tell me. the soles of the feet. They will say that they are hiking or camping in the Rocky Mountains. They may
even give you a mountain range, so don't even be surprised about that, okay? Perfect.

03:17 How do we treat Rocky Mountain Spotted Fever? Yup, we treat it with doxy. So
we treat it with doxy, good. Make sure that you know presentation and treatment. This is part of those
buzzwords. This what we're focusing on for this exam. If they say "Middle-aged lady presents with...Can
you identify what the disease process is?" So, let's try this again.

03:46 Middle aged lady comes in. She tells you, "I've been out in the yard. We have a lot
of high grass and things like that. And we were just out there having a good time. And I found a tick on
my body. And now look at this thing on my leg."

04:03 Now, you look and you see a target bullseye rash. Key finding, right? What do you
think that is?

04:11 Of course. It's Lyme Disease because we're always going to go with that...target
bullseye rash. And what else did she say? What's that other buzzword? She said she found a tick on her
body, right? So target bullseye rash, found a tick, boom. That's got to be Lyme's Disease.

04:27 Now, here's the kickers. What's the other name for Lyme's Disease? Erythema
migrans, right? If it has two names, we have to know that different name. Because I like to say
"Synonyms."

04:44 This test likes synonyms, so I'm going to be pointing some out, especially if you
are in the live session. I'll be pointing some of those synonyms out.

04:52 Lyme Disease. [inaudible 00:04:55] by rash, found a tick on her body after she
came in and a couple days later developed that. Good.

05:05 How do we treat Lyme Disease? Very good. We also treat that with doxy. Good
job.

05:12 Middle-aged man comes in and he asks you to look at this skin lesion. And this
skin is on his arm. It's on his arm and you look at it and it has got irregular borders, it's got different
colors, it's got this light brown, dark brown. And it looks like there's some black in here. And you're
looking at it. Your spidey senses are going off. So, what is that?

05:40 It's melanoma. Again, irregular borders. Text book, right? We all discussed
melanoma even when we were back in nursing school. Irregular borders, dark colors, usually three
different dark colors, like a dark brown, a light brown, and maybe a black or a dark, dark brown.
Something like that. Very good.

06:03 That is melanoma.

06:04 A fairly young woman comes in to see you. And she's like "Something's going on
here." And you look at her skins and it's just red. It's red. She seems to have these plaques on it. Little
silvery plaques. It is itchy, when you ask her, she says, "Yeah, it's kind of itching a bit. What's going on
there? What is that?"

, 06:33 Oh yeah, it's psoriasis. Silvery, which scales. I always like to tell people, think of
the commercials that are out there right now. When you're looking at the commercials, what's
happening? The skin is red. There's some silvery white scales. I think one even has ants growing on
someone's hands... Not growing, but running on their skin. Because it's itchy. So all of that. That is
psoriasis.

07:06 Now, let's say that we have psoriasis and this middle aged man comes in and he's
like, "Ooh. I fell. I scraped up myself really really good." And he's growing that scab. And you look at that
scab, and over that scab that's forming, what do you see? You see some psoriatic plaque forming. What's
going on there? What is that called?

07:34 It's called Koebner phenomenon. That's what happens. Basically, there's a trauma
and somebody hurts themselves and a scab forms just like any other place, like if you skinned your knee.
The scab forms. But over that scab, the psoriatic plaque starts forming. That's what that's called. It's
called Koebner phenomenon.

07:55 If we take one of those plaques and we scrape at it, flake if off a little bit, what is
that called? And it starts that pinpoint bleeding? What is that called?

08:07 Of course, it's called Auspitz. Auspitz Sign. If you take a psoriatic plaque and it
starts that pinpoint bleeding, that is what that's called.

08:19 We're going to take a pause right here just because I want to mention some
things. Now that you see how this is going, you notice I'm very conversational. So if you want that stuffy,
old, teachering, professorship kind of situation going on, that's not me. That's what you're going to get.

08:43 This is supposed to be fun. Being a nurse practitioner is exciting and I love it. It's
fun. So it doesn't have to be all stuffy. We learn all types of different ways. And let's be real about it.
When we're reading in a book and it's those straight words, or our professors are lecturing who are not
having a good time, it's a little boring, right?

09:07 So my idea is to give you these buzzwords but make it fun, okay? Let's make it fun
so we can remember these things so when we go to take the test, we remember what we read or heard
or anything like that.

09:25 Let's say Mom brings in her five-year-old child and she's like, "This child keeps
complaining about itching. I've done pretty much everything. It's mostly happening at night when he's
really complaining of itching." You open his hands, you look in there, and you see that burrowing. What
is going on?

09:47 Oh yeah, that is scabies, right? If you've ever seen scabies, you can actually see
the burrowing that's happening under the skin. It always gives me the willies and the chills. That's what's
happening.

09:57 What instructions am I going to give Mom? And what medications, also, am I
going to give? These are the things that you need to know for this exam. You need to give Mom the
Permethrin cream. That's one. I think we all remember that. And typically, we are going to treat
