Chapter 2 legal and ethical issues: respecting client rights (mental health)
-Pt who have mental health disorders diagnosis or who are receiving acute care of mental health
disorder are guaranteed the same civil rights as any other citizen. Which may include
The right to humane treatment and care, such as medical and dental care, informed consent and
the right to refuse treatment, and confidentiality
-A written plan of care/treatment that includes discharge follow up as well as participation in the
care plan and review of that plan
-Communication with people outside the mental health facility, including family members,
attorneys, and other health care professionals.
-Beneficence: doing good ex) nurse helps new pt who has a psychotic disorder to feel safe in the
environment. Autonomy the clients right to make her own decisions but the client must accept
the consequences ex) rather than giving advice to the client nurse helps pt explore options
Chapter 29 crisis intervention for intimate partner abuse (mental health)
-Types of crises situational/external can be unanticipated loss or change experienced everyday
“life events”, maturational/internal can be achieving new developmental stages, adventitious the
occurrence of natural disasters
-Phases of a crisis phase 1 escalating anxiety from a threat activates increased defense responses,
phase 2 anxiety continues escalating as defense responses fail, functioning becomes
disorganized, and the client resorts to trails and error attempts to resolves anxiety, phase 3 trial
and error methods of resolution fail and the clients anxiety escalates to severe or panic levels
leading to flight or withdrawal behaviors, phase 4 the client experience overwhelming anxiety
that can lead to anguish and apprehension.
-common crisis characteristics can be sudden event with little or no time to prepare, perception
of the event as overwhelming or life threating, loss or decrease in communication with
significant others, sense of displacement from the familiar and actual loss
Chapter 12 Informed consent disorders of the eye; evaluating understanding of cataract removal
neurosensory disorders (medsurg)
-Cataracts can be determined upon examination of the lens using an ophthalmoscope. Cataract is
an opacity in the lens of an eye that impairs vision
-common causes are age-rates (dry eyes), traumatic (injury), toxic (meds betas, steroids,
thiazine), associated (diabetes, sun exposer, hypoparathyroidism, DS), complicated (intraocular
disease ex glaucoma or retinal detachment)
-medication pt take is anticholergics (atropine ophthalmic) fast acting but effects can last up to 7
days

-audits of quality control can produce valuable quantitative data the types are structures audits
evaluate the influence of elements that exist separate from or outside of the client-staff

-Pt has spina bifida must assess child for allergies such as latex, they have an increased risk.
-need to educate client to avoid items such as water toys, pacifiers, plastic storage bags, trash
bags, bananas
-Educate pt families of S&S of an allergic reaction and when must they report it to provider,
instruct on how to use epinephrine
Chapter 12 bacterial meningitis (pedi)
-infection caused by bacterial agents Neisseria, streptococcus pneumoniae, haemophilus
influenzae type B.
-incidence of bacterial meningitis has decreased in all age groups except infants under age 2
months since PCV vaccines have been given.
-when you suspect meningitis you need to isolate client as soon as possible when will be droplet
precautions need to have/ maintain this for at least 24 hours after antibiotic therapy
Chapter 4 how to manage heart burn (maternity)
-pt should eat small, frequent meals, not allow the stomach to get too empty or too full, sit up 30
minutes after meals.
-check and ask providers before tanking antacids
-no spicy, greasy, fried foods and drink plenty of fluids between meals
Chapter 25 understanding formula (maternity)
-bottle feeding can be successful and adequate source of nutrition in the mother chooses not to
breastfeed. New born may need to eat every 3-4 hours
-formula may only be refrigerated for up to 48 hours, can mix powder with tap water if the water
is questionable should be boiled 1st.
-bottle feed is best at 45 degree angle help close to infant
Chapter 6 teaching older adults about vaccines (medsurg)
-vaccines recommended for older adults are:
-annual flu shot
-pneumococcal disease and herpes zoster (shingles)
