NR 500 Week 2 Reflection on Learning
Reflection Week 2
This week I learned a little bit of history of where the role of advanced
practice nursing comes from. I learned that the idea started back in 1965 with nurse
Loretta Ford and Dr. Henry Silver in Colorado. I was under the impression that this
idea of a nurse practioner started more recently, but to know it was was initiated in
1965 really surprised me. As well to know how many years it has taken to develop
the proper program and yet there is still the debate of what qualifications does a
nurse practioner need to be competent. There are so many aspects to this new role
of advance nursing and how it created confusion for many where they refer to the
NP as “playing doctor” as listed on our week 2 reading lesson. Being a registered
nurse on the medical surgical floor I have been confused many times when a female
with a white lab coat has just seen my patient and I refer to her as Dr. what is new
with the patient, and they correct me that they are not the doctor, but nurse
practioner for a particular physician. In my practice I took care of an elderly female
patient whose daughter was in the medical field and when her mother was seen by
the nurse practioner, she then mentioned to me how she feels the NP’s don’t know
anything compared to the physician and that came to be as a shock. Automatically
the daughter of my patient felt like her mother was not getting the proper care since
an NP was rounding on her mother that day rather than the physician. This
perception and gap must be bridged so patient care does not suffer.
Moving forward, through my research for the paper this week, I learned in
depth of the different roles in advanced practice nursing. I briefly knew before
selecting the FNP track, but this weeks writing assignment really helped me learn
exactly what goes on in each role of APN. After all the research, I am still firm that I
chose the right patch of advanced practice nursing as I feel I will be best fit as a
family nurse practioner considering my desire for general medicine bedside patient
care.
• What do you most look forward to in your new role as an NP?
In my new role as an NP I am most looking forward to developing the critical
thinking skills necessary to aid in diagnosing a patient health issue and having
the autonomy to make the necessary decisions for my patient as appropriate.
As a registered nurse at times I have recommendations that can help my
patient, but doctors don’t always follow through on time for example for redness
in the groin area and I am requesting miconazole powder and I am not getting
the order from the physician, but I am helpless because I cannot put my own
orders in. Often time these little things get pushed off, but they lead to bigger
skin issues for the patient.
• What concerns do you have about transitioning to the role?
As listed in my previous reflection response, I am concerned mostly about the
gap that patients and families often have when a nurse practioner rounds
