Stages of labor
• First Stage - labor- variable
has three phases latent, active, and
transition
Begins with onset of labor though
10cm dilation ends with (complete
dilation)
longest phase 12.5 hrs.
latent phase 0-3cm begins with cervical dilation 0 ends
with cervix @ 3cm
o Cervix 0 cm
o Irregular, mild to moderate contractions
o Frequency 5 to 30 min ›
o Duration 30 to 45 seconds
o 4-6 hours
Objective signs:
,Modules 3 ,4 and 5 (Labor, Postpartum and Newborn care)
o cervical change
o some effacement
o communicative- eager to talk
o Contractions Irregular, mild to moderate
-Frequency: 5 to 30 minutes
-Duration: 30 to 45 Seconds
Cervical dilation 1 cm/hr. for clients who are
primigravida, and 1.5 cm /hr. for clients who are
multigravida, on average
active 4-7 cm- Intensity in contractions. 2-3 hours
Begins with:
o Cervix 4 cm
o More regular, moderate to strong contractions
o Frequency 3 to 5 min
o Duration 40 to 70 seconds
Ends with
o cervix 7cm dilated
Objective signs:
o bloody show - This is where we medicate mom
o Contractions are getting closer together and
stronger.
o Cervical Dilation (4 to 7 cm)
o Contractions more regular, moderate to strong

o Rapid dilation and effacement
o Some fetal descent
o Feelings of helplessness
o Anxiety and restlessness increase as contractions
become stronger
Membranes may rupture, contractions 3-5 min apart, more focused, less talking,
risk to baby is greater, amount of perfusion to baby is less, baby can get hypoxic
during long labor with progression
Transition 8-10 cm very strong contractions 20-40
minutes
Begins with:
o Cervix 8 cm
o Strong to very strong contractions
o Frequency 2 to 3 min
o Duration 45 to 90 seconds
Ends with:
o Complete dilation of 10cm
Objective signs:
o Tired
o restless, and irritable
o Feeling out of control, client often states,
“cannot continue”
o May have nausea and vomiting
