Student will perform physical assessment, and health screening, then document findings on assessment form. Clinical mapping assignment must be completed using the content from the assessment and health screening. Assignment Directions 1. Student will complete physical assessment on patient 2. Assess patient’s records of pertinent labs. 3. Assess and document all pertinent medical history 4. Assess patient’s records for home and inpatient medications and rationale for usage 5. Assessment of demographic data. 6. Identify any treatments. 7. Identify admitting medical diagnosis 8. List all actual and risk for nursing diagnosis on your patient on separate sheet of paper in order of priority. 9. Identify supportive data for selected priority nursing diagnosis. 10. Create short term goals (STG/NOC) for the priority diagnosis 11. Create nursing interventions with rationale for the priority diagnosis 12. Identify if goals are met/unmet and any needed modifications. 13. Complete mapping template, identify any patient/history/content links you may note 14. Include all pertinent references and citation
