Piya Jordan Allergies: cephalosporins, penicillin sensitivity, milk intolerance – gets upset stomach Physician Orders Mon 2115 EKG Orthostatic blood pressure readings Continuous cardiac monitoring Oxygen 2L by nasal cannula Labs: CBC, urinalysis, electrolytes, lipase, amylase, LFTs, PT/INR, digoxin level Chest Xray PA and lateral, KUB and upright IV normal saline one liter bolus Mon 2200 Meperidine 50 mg with Promethazine 12.5 mg IV push x 1 dose Potassium chloride 20 mEq in 250 ml NS to infuse IV over 2 hours Mon 2320 STAT CT of abdomen Tues 0015 Foley catheter to gravity drainage Nasogastric tube to low continuous suction Meperidine 50 mg with Promethazine 12.5 mg IV push x 1 dose Admit – Surgery service. Diagnosis: colon mass, rule out bowel obstruction Tues 0130 Promethazine 12.5 – 25 mg IV every 4-5 hours prn nausea Acetaminophen 650 mg per rectum every 6 hours prn fever 102 F Labs: Hemoglobin and Hematocrit Chem 7 PT/INR Type and crossmatch for 2 units Diet: NPO Vital signs every 4 hours Intake and Output every 8 hours Oxygen 2L flow by nasal cannula Telemetry monitor Activity: bedrest with bathroom privileges Nasogastric tube to low continuous suction Foley catheter to gravity drainage Mineral oil enema
