Mechanical falls in the emergency department can be minor, not requiring hospitalization, no injury or

skin tears that can be repaired, or major, that require hospitalization such a fractured hip of a brain bleed.
There is also the occasion that the fall may be bad enough that it could result in death immediately or
while hospitalized.

P: I will be looking at patients that enter the emergency room to identify those that are a fall risk with a
fall risk assessment that is appropriate for an emergency room. The falls that will be focused on will be
mechanical falls not intentional falls such as acting out.

That approx. 6% of the falls within a hospital happen in the emergency department and can lead to poor

patient outcomes or hospitalizations (Pop, 2020). Much of the known fall risk score interventions are not
made for an emergency room. When triaging someone that comes in via private car do you truly know if
they use a commode or urinal at home or if they use any assistive device for ambulation because everyone
comes in by wheelchair. Patients are not great historians and when you ask them the medications that they

take they say “a pee pill, a heart pill and the little white one” medications can cause a patient to need to

get out of bed often for the bathroom or can cause dizziness if they get up too fast which they may not
know about. When a patient is taken to another department for testing, they may not have any indications
the patient are a fall risk.

By changing the fall risk to questions that a nurse can use some subjective data such as gait pattern with
or without assistive devices, even a change to the type you use in the ED most use the same fall risk
assessment tool as the inpatient setting which has a much more controlled setting. The other option that
would be some type of indicator on the patient that would indicate to all staff the fall status. The second

one may be a bit more of a challenge with management as they at one time had put out notifications that
you could not differentiate one patient from another by giving them a different color sock or band
indicating they were a fall risk.
