Focus of the question: Many patients do not understand the importance of following a regimen to help reduce heart failure
symptoms and by providing home health care patients can have decreased hospitalizations. The
nurse’s in home health aim at monitoring and teaching the patients and caregivers signs and
symptoms of congestive heart failure before having recurrent visits to emergency room.
