problems since adolescence. Weight gain related to poor diet high in fats and carbohydrates.
Herniated disc: Patient has a herniated lumbar disc as documented via MRI. Patient received
intraspinal corticosteroids and physical therapy 12 months ago. She had good results in that her
pain resolved and she did not have any sensory or motor deficits. Pain from this disc increases
with weight gain and lack of low impact exercise, e.g., walking.
