Introduction
The healthcare system like any other sector has received immense changes in the last couple
of years with regard to treatment and intervention strategies. The growth in the industry has
resulted in a growing aging population which is a vital indication of healthcare efficiency but in
turn, it presents major challenges for the healthcare professionals. It is without a doubt that the
aging population is highly susceptible to chronic illness because their immune is defected and
thus unable to fight even minor infections. According to Watts, Zayed, Llewellyn-Thomas &
Schnurr (2016), with respect to Veterans, Post Traumatic Stress Disorder (PTSD) and Traumatic
Brain Injury (TBI) are common mental illnesses. However, the treatment measures have
improved when compared to past decades but there is still a challenge when it comes to treating
the aging population that has lived with the illnesses for years or decades but on their health
complexity. The use of Population Health Improvement Initiatives (PHIIs) was mainly initiated
in the quest of reducing the associated health cost while increasing health stability across diverse
population groups such as elderly, pregnant women, ethnic minorities and so on. Mr. Nowak
suffered from TBI after a severe fall and has been complaining of the loss of balance. Evidence-
based Safe Headspace was recommended as the most cost-efficient Health improvement
initiative that works in improving health outcomes of Elderly individuals suffering from TBI and
PTSD. The objective of the present evaluation is to establish knowledge gaps with relation to the
PHIIs, create strategies to bridge the existing gaps and integrate new intervention approaches
into Nowak’s care plan. The acquired evidence from the evaluation will be useful in devising an
assessment framework for patient care.
