Patient Centered Care Report
Health care initiatives for large groups are beginning to shape the influence of health care
today. Population health is one of the focuses of the Institute of Medicine’s healthcare initiatives
for America. Population health improvement initiatives (PHII) were introduced to increase the
health of a desired population, while decreasing costs both to the consumer and the company.
PHII’s provide detailed information on patient-centered care, and cost in the form of measurable
data, including clinical and economical outcomes. Healthcare professionals consider the
information gathered by PHII’s to plan care for individuals and groups (Huber, 2017).
Evidence that a healthcare professional may encounter may not be specific to the exact
need of the patient being seen. It is in this case that the healthcare professional may require the
use of information gained by PHII’s and use this evidence to develop a plan of care specified to
the individual patient. Contextual transfer of information is discussed in the case of Mr. Nowalk,
at Uptown Wellness Clinic, regarding his traumatic brain injury(TBI) after a fall. Mr. Nowalk has
complained that he has been having difficulty with his balance, and attributes this to his TBI.
Evidence from the PHII, Safe Headspace, can be used to develop the plan of care for Mr.
Nowalk, based on evidence from the patient care group having TBI’s and Post-traumatic stress
disorder (PTSD) (Capella University, n.d.). The evidence gained from Safe Headspace’s program
can be applied to Mr. Nowalk’s case and used to develop a plan of care specific to him.
Evaluation of a Population Health Improvement Initiative’s Outcomes
Safe Headspace has been proven that they are able to improve the outcome of care for
elderly patients diagnosed with PTSD due to TBI. Safe Headspace treats patients with the use of
