Psychiatric Assessment: 32-Year-Old Latina Woman with Major Depressive Disorder
Subjective Data
Chief Complaint (CC): “I am feeling depressed”
History of Present Illness (HPI): PM is a 32-year-old Latina female presents at the facility in
the company of her husband with complaints of persistent loss of interest and sadness. The
husband notes that the disorders began two weeks ago and has influenced the way she behaves
resulting in various physical and emotional problems that affect her daily activities. He notes that
her problem is mental related and is characterized by feelings of sadness, loss of pleasure in most
activities including sports and coitus, sleep disturbance, anxiety, and headaches. She notes her
manifestations happen most of the day, nearly every day affecting social activities and work. She
experiences outbursts of frustration irritability, or anger, even over small issues, and feels
worthless fixating on past non-fulfilments. She has regular death contemplation and often wants
to stay home, rather than going to work or out to meet with friends. She notes that the condition
is worsening and rates its severity as seven (7) on a scale of 1 to 10.
Past Psychiatric History: She notes baby blues 3 years after giving birth to their last-born child,
but with no hospitalization.
Substance Use History: She denies any history of alcohol abuse and dependency on any
substance drug.
Past Medical/Surgical History: She notes two caesarian sections without any complication.
Current Medication: The patient is currently under no prescription medication or OTC
medication.
Allergies: denies food allergy, denies allergy to pet far, notes penicillin rash
