Most medications designed to be absorbed orally have a
-Doesn’t inhibit transpeptidase (like Vancomycin molecular weight of 400dalton but Vanco has 1450Da !
penicillin) ,but it sticks on peptidoglycans Can’t pass blood brain barrier or deep tissues.
-Acts on G(+) especially cocci Vanqueur
Very rich in -OH group -> water soluble , not
absorbed orally and Depend on renal excretion
-Never used as 1st choice for staph
Indications Side effects
Uses : 2nd choice after fail of other ATB VAN
Vein: Thrombophlebitis (most common)
Diluted in 500mL over 60min infusion
Allergy: peptide causes histamin
MRSA C.diff pseudomembranous colitis release Red man syndrome upper half of body
Nephrotoxic (although not proved)
Must be given as IV infusion with gentamycin 2nd choice after metronidazol
never given alone
Staph Infective endocarditis : start with floxacillin if
fail give cephalosporins 3rd generation Then if fail
