responsible for documenting the advance directive in the patient chart. The advance
directives must reflect the client’s decisions. The nurse is also responsible for informing the
family about the client’s decisions. Lastly the nurse should inform all members of the client’s
advance directives.
What safety precautions should be implemented when a client reports an allergy to latex?

allergies. If the client reports a latex allergy the team should use latex free gloves,
equipment, and supplies.
A nurse is caring for a client who is vomiting. What are signs and symptoms of dehydration?

Creatinine labs, urine output of >30 ml/hr., dark urine, dry mucous membranes, sunken
eyes, and dry cracked lips.

Proper oral hygiene helps decrease the risk of infection for clients. They should receive oral
hygiene at least once a day. In my clinical experience, we provide oral hygiene at least
every 2 hours while they are on a ventilator.
