If I were the primary nurse caring for Mr. Bates, there are several things I

would have done differently during the SBAR interaction with his current
nurse, beginning with the report conversation. While it is important to
include patient’s in their care, I feel that because of the nature of the
conversation, it would have been more appropriate to discuss Mr. Bates care

somewhere privately, away from the patient. I also found it very concerning

that Mr. Bates current primary nurse made the decision to disregard hospital
policy for that particular unit regarding patients who are receiving vasoactive
IV infusions, because she assumed it would be more “stressful” to move him
to a more qualified unit, prior to being transferred to the Cath lab. Hospital

policies are put in place to protect our patients and the medical staff and
disregarding those policies can sometimes lead to potentially life-threatening
situations for our patients. Because of this I would have consulted with Mr.
Bates physician regarding his current condition and explained the need for a
unit transfer until the patient could be moved to the Cath lab. Afterwards, I
would have rounded on the patient and provided an update regarding the
plan of care for him, so he is aware of what is going to occur and why.
