Its focus lies in the provision of recovery-focused and collaborative services to connect people to healthcare services (Hannigan et al., 2018). A primary necessity for care coordination is the management of chronically sick patients. It creates a challenge of the illness to personal identity and alteration to social relations and creation of uncertainty. It is even more worrying since an effect to one sphere of life may affect the other (Hannigan et al., 2018)
