The AACN and the international nurse council, defines the advanced practicing nurse (APN) role as an individual who has obtained the advanced knowledge, training and education to expand their clinical knowledge and role by earning an advanced degree in nursing. The specification of their training and skills is what makes them differ from other registered nurses. If focus and training is for the role of nurse practitioner, the national certification exam must be taken after graduation. They must also obtain a special license issued by the state in which they will practice. The role of the APN includes certified nurse practitioners (CNP), the certified nurse midwife (CNM), the clinical nurse specialist (CNS), and the certified nurse anesthetist CNRNA). (Denisco, Barker, 2015) The roles differ with each specialty of APN and the guidelines for APN practice is different from state to state. Certified Nurse Practitioner Certified Nurse Practitioners (CNPs) are advanced practice registered nurses who provide health care and disease process management. A nurse practitioner assesses the needs of the patient, order lab test and interpret diagnostic studies. (Abel, L. (2018) CNPs also diagnose illness and disease, they prescribe medications as well as formulate treatment plans. CNPs provide care in a multiple settings, including but limited to primary care, schools, acute care hospitals, or long-term care facilities. In most states, nurse practitioners are recognized as independent providers and can prescribe medications and other necessary treatments for patients. Training of the CNP includes basic disease prevention, care coordination, and health promotion. According to the AANP, a nurse practitioner has graduate level education and must complete a clinical requirement to provide the specialty care depending in their field of practice. The CNS has an advanced knowledge base of disease processes in assessments, diagnosing, and treating illnesses, however the role of the CNS extends far beyond just providing patient care. The CNS may function in role of an independently licensed provider, as a clinical expert or as a nurse educator in the acute care setting. Some places may restrict the CNS to the role of the nursing educator, and others will use the CNS to the highest level of their clinical expertise. In this role the CNS works as a quality improvement specialist, implements evidence-based practices, and as a clinical practitioner in varied settings. The CNS programs of study also include and master’s degree from an accredited institution as well as a clinical experience and preceptorship that require clinical hours. Certified Nurse Anesthetist The role of the Nurse Anesthetist is defined as an advanced practice nurses who administers anesthesia for medical procedures as well as surgery. The CRNA partners with surgery MDs, anesthesia MDs, and other professionals to provide anesthesia for procedures. CRNAs provide pain management, assist with stabilization services, and oversee patients in recovery. CRNAs are the primary providers of anesthesia in rural and medically underserved areas of the U.S. CRNAs also provide care for dental patients, podiatrist and in other medical settings. CRNs are trained advanced practice nurses with an advanced degree from an accredited institution that includes coursework and a clinical requirement. Becoming an CRNA takes several years of advanced education. They must also take and pass an examination for certification. CNMs practices in multiple settings, including but limited to hospitals, private practice, medical clinics, birthing centers, attend at-home births, and public health systems. Their job description is to assist with births, promoting reproductive and primary care education, offering counseling, and ensuring the health of women and their babies. The CNS role also involves providing sex education and health. Nurse midwives also see women for routine exams and can initiate contraception. (McClellan 2014) Certified nurse midwives can prescribe medications, treatments, and apply therapeutic and diagnostic measures. CNMs can provide care to females of all ages and stages from infancy to elderly. This may also include care for their newborn, antepartum patients, intrapartum patients, postpartum patients, and nonsurgical gynecological care.
