1. Room 2301 Susan: 93-year-old who fell at home and is waiting for nursing home
placement. Uses a walker & needs assistance to the bathroom & ADLs.
2. Room 2312 Fredrick: 57-year-old recovering from a knee replacement who
needs educational reinforcement and is going home today.
3. Room 2319 Joshua: 63-year-old in for cardioversion and is going home today.
4. Room 2329 Brian: 49-year-old diagnosed with HTN and Alzheimer's Disease who
wanders off the unit & needs assistance with ADLs.
5. Room 2304 James is a 95-year-old who has osteoarthritis and needs help getting out of
bed, bathing, & dressing.
6. Room 2307 Francis is a 39-year-old being discharged today. Received discharge teaching
yesterday about her ruptured appendix wound & wants to review the care instructions before
she leaves.
Acuity Level 2 – 8 Patients
1. Room 2302 Brooke: 33-year-old admitted with new-onset DM & uncontrolled blood
sugars. Started on new medications, including insulin, & needs additional patient/family
education.
2. Room 2311 Mark: 83-year-old who is on isolation for MRSA and is on IV antibiotics and
ten other medications.
3. Room 2320 Jeffrey: 63-year-old non-compliant with meds and came in with HTN
and crackles in his lungs. Daily labs & wound on left foot.
4. Room 2330 Leslie: 46-year-old had unstable angina and had a cardiac
catheterization yesterday late afternoon and stayed for observation.
5. Room 2305 Traci: 72-year-old who has a history of heart disease, a history of
myocardial infarction, and mild dementia.
6. Room 2308 Alecia: 33-year-old who was experiencing chest pain and is scheduled for a
stress test later today.
7. Room 2303 Theodore: 22-year-old has DM since 10-years-old who requires
med adjustments. (med adjustments can only be done by RN)
8. Room 2318 Bruce: 45-year-old who was placed on isolation for C-Diff and is on several
IV mediations.
Acuity Level 3 – 9 Patients
1. Room 2306 Amelia: 83-year-old after a stroke and requires total care.
2. Room 2313 Audrey: 78-year-old newly diagnosed COPD. (will need teaching – only an RN
can teach)
3. Room 2317 Abigail: 90-year-old diabetic with a chronic leg infection.
4. Room 2328 Roxanne: 82-year-old newly diagnosed lung cancer. (will need teaching –
only an RN can teach)
5. Room 2322 Nancy: 56-year-old chronic kidney disease & on peritoneal dialysis. (just
needs vitals monitored – CNAs can do this)
6. Room 2323 Dennis: 35-year-old admitted with right upper quadrant pain that radiates to
the right shoulder.
7. Room 2314 Roger: 46-year-old is wasted and malnourished. She has severe diarrhea.
8. Room 2309 Terry: 59-year-old has bladder cancer with painless hematuria.
9. Room 2310 Barry: 54-year-old has a history of mitral valve regurgitation.

7. Room 2316, David: 54-year-old with weakness & a decreased level of consciousness.

Dear Nurse Manager,
Due to our staff shortage, a new staffing assignment has been completed based on the
patients' acuity level on our unit. The assignments were made based on the staff's scope of
practice and competency to ensure all patients receive the care they need and the quality they
deserve.
For the six patients with an acuity level 1, their needs will be met by the LPNs and CNAs.
Each LPN is assigned to one, and each CNA is assigned to two. Because these patients stable and
their health needs are predictable and manageable, they are appropriate for licensed practical
nurses and certified nursing assistants (Aiken et al., 2018). The eight patients with an acuity level
