The Reason for Referral and Background Information
Jane was referred for evaluation by the school psychologist due to recent behavior
problems and declining academic performance. Jane was referred to the school
psychologist for an evaluation due to having emotional outbursts in the classroom and the
lack of academic progress in the most recent 6 week period. The school records indicate
that Jane had five excused absences due to illness so far this year, and no unexcused
tardies. Jane has been referred for in-school suspension three times for behavioral
outbursts in the classroom. Her grades mostly consist of C’s and she is currently failing
two classes. Her writing and reading skills are above average for her age, and she works
better when she works directly with teachers rather than her peers. Records show that
Jane is up to date on all of her vaccines that are required, she has completed both vision
and hearing testing, and her well-being is within what is considered a normal range. Jane
has reported that she has been having trouble with bullying from her peers and often feels
sadness. Her parents have given information pertaining to her progress, family history,
school history, and behavior at home. They have indicated that she has intentionally
, missed curfew several times. According to her parents she is honest and gets long well
with other children, including her younger brother. Frustration is also comes easily to her.

BASC-2 assessment
Cognitive Affective Personality System (CAPS) model
The test that are used for this assessment are the Minnesota Multiphasic Personality
Inventory- Adolescent (MMPI-A), Mental Status Examination, the BASC-2 Assessment
and the Cognitive Affective Personality System (CAPS) model. These tests are
commonly used when it comes to assessing individuals. “The Minnesota Multiphasic
Personality Inventory-Adolescent assessment is generally suitable for contemporary
adolescents, and item content appropriately reflects adolescent personality and
psychopathology” (Claiborn, et al, 1995). It is one of the most used assessments among
adolescents. While it can measure Jane’s mental health, it can also provide evidence of
the mental disorders that she may currently have. “The test was published in 1992 and
consists of 478 true/false items” (Leeth, 2011). There are different scales that are covered
in this assessment that are known as Hypochondriasis, Depression, Hysteria,
Psychopathic Deviate, Masculinity-Femininity, Paranoia, Psychasthenia, Schizophrenia,
Hypomania, and Social Introversion. A mental status evaluation being completed will
allow for the Doctor to see how the client expresses their thoughts and emotional state.
