Introduction of Community (20 points) Identify the city and state of your community and briefly describe the community you will be using for this assignment. It should be the area where you live or the area surrounding your work setting but must include a residential area. Demographic data are not needed. Grand Island NY – An Island between Niagara Falls and Buffalo looking on Canada for over half the western edge. It’s a quick 10 min drive to Buffalo or Niagara Falls. This is the town that we are raising our children. 2. Windshield Survey (75 points) a. Vitality: Use a majority of the questions from Box 6-2 in your text to describe your observations about the community vitality. • Are people visible in the community? What are they doing? There are a few small shopping plazas on Grand Island. Some are just half filled with businesses. There is a small family owned amusement park. These are places you will see your neighbors. However, there are not many places of business on Grand Island so most resident commute off the Island for employment. This also does not allow for much visibility as in business district of other towns. • Who are the people living in the neighborhood? What is their age range? What is the predominant age (e.g., elderly, preschoolers, young mothers, or school-aged children)? - Age Range – 65 % of the population being between the age of 18-65 yrs of age. The is a large family presents on the island. • What ethnicity or race is most common? - 76.9% of the population identify as White - 13.3% of the population identify as African American - 17.8% of the population identify as Hispanic • What is the general appearance of those you observed? Do they appear healthy? Do you notice any people with obvious disabilities, such as those using walkers or wheelchairs, or those with mental or emotional disabilities? Where do they live? - Grand Island is located in Erie County which is ranked 50 for Health Outcomes with a life expectancy of 59 yrs of age. 14% of which consider themselves to be of poor or fair health (on par with the NYS average) with an average of 3.7 poor physical health day and 3.8 poor mental health days. My personal opinion is that most of this community seems to be of good health. However, we do see may individuals in the community that have disabilities and we do have a baseball diamond for the handicapped to play called “Miracle Field”. This allows for increased visibility. • Do you notice residents who are well nourished or malnourished, thin or obese, vigorous or frail, unkempt or scantily dressed, or well dressed and clean? - Most of the community seems to be average to well-dressed but not overly pretentious. - Most seem to be ranging from average weight to overweight. • Do you notice tourists or visitors to the community? - We have a large sports park with many basketball, tennis, baseball, softball, football and soccer fields. We also have a State park with a small beach with and other outdoor activities and a small family owned amusement park. These venues attract people from out of town on a regular basis. • Do you observe any people who appear to be under the influence of drugs or alcohol? - We have event in the center of the island that has open drinking abilities but outside of this and the bar scene nothing out of the ordinary. • Do you see any pregnant women? Do you see women with strollers and young children? - May of the venues we have are family friendly and the area has had a 4 % growth in children under the age of 18. b. Indicators of social and economic conditions: Use a majority of the questions from Box 6-2 in your text to describe your observations about the social and economic conditions. •What is the general condition of the homes you observe? Are these single-family homes or multifamily structures? Is there any evidence of dilapidated housing or of areas undergoing urban renewal? Is there public housing? What is its condition? - The majority of the homes on the island are single family residences. We have had many new building companies in the area recently which has brought more apartments, condos and duplex style homes that were not available and in high demand. • What forms of transportation do people seem to be using? Is there public transit? Are there adequate bus stops with benches and shade? Is transportation to health care resources available? - Most people drive person vehicles. You will see the occasional taxi. There is a bus system but it is only available on the main boulevard. • Are there any indicators of the kinds of work available to residents? Are there job opportunities nearby, such as factories, small businesses, or military installations? Are there unemployed people visible, such as homeless people? - The are a few office buildings and factories on the island but only a few of note. The rest of the work is mainly the schools, restaurants, and entertainments venues. The is virtually no homeless visible. • Do you see men congregating in groups on the street? What do they look like, and what are they doing? - Unless there is an event there are not “congregating” on the streets. • Is this a rural area? Are there farms or agricultural businesses? - There are a few farms on the island but most at self sufficient not for mass produce. • Do you note any seasonal workers, such as migrant or day laborers? - There are none on Grand Island but there is in the area north of Niagara Falls in the Wine and Apple Farms. So many of the resident of the Island are familiar with them. • Do you see any women hanging out along the streets? What are they doing? - No • Do you observe any children or adolescents out of school during the daytime? - No • Do you observe any interest in political campaigns or issues, such as campaign signs? - We are a very divided population on the political front but very active in the polls. Many houses and business have signs of support for local, state and federal politics. • Do you see any evidence of health education on billboards, advertisements, signs, radio stations, or television stations? Do these methods seem appropriate for the people you observed? - With Buffalo so close, we have build boards advertising many different types of health insurance, hospitals, health care systems and health care venders. • What kinds of schools and day care centers are available? - We have lottery system for Pre-K and 3 different large daycare facilities on Grand Island. We have 3 elementary schools and a large Middle and High School Campus. c. Health Resources: Use a majority of the questions from Box 6-2 in your text to describe your observations about the health resources. d. Environmental conditions related to health: Use a majority of the questions from Box 6-2 in your text to describe your observations about the environmental conditions. e. Social functioning: Use a majority of the questions from Box 6-2 in your text to describe your observations about the social functioning. f. Attitude toward healthcare: Use a majority of the questions from Box 6-2 in your text to describe your observations about the attitudes toward healthcare. 3. Vulnerable population (20 points) Describe a vulnerable population or aggregate that you identified during your survey. What did you observe about this group such as their appearance and actions? 4.
